Dialysis Clinic, Inc. Dialysis Social Worker-MSW in Concord (Walden Pond), Massachusetts


The Dialysis Social Worker is responsible for assessing and responding to the psychosocial needs of our in-center hemodialysis patients. This includes counseling patients and their families on ESRD related issues consisting of treatment options, adjustment to chronic illness, insurance needs, and community resources.

This position is a part-time position, 20 hours/week, with flexible days/hours .


  • Responsibilities include:

  • Counseling patients and their families regarding End Stage Renal Disease therapies. Assisting patients in comparing the different therapies available so they are able to make educated decisions about their own care.

  • Provide information regarding insurance coverage so that a patient can maximize their insurance coverage for treatment. Assist patient with completing insurance/financial forms.

  • Assess patient’s personal and homecare needs so that referrals can be made to community and transportation agencies to maximize patient’s ability to access care.

  • Social Worker is part of the interdisciplinary team providing ongoing assessment of patients’ needs and modifying the patient’s plan of care.


  • Masters Degree from an accredited Social Work Program.

  • Current clinical license in the state as a Social Worker.
