Dialysis Clinic, Inc. REACH Administrative Support in Nashville (Corporate Office), Tennessee

Overview

REACH Kidney Care is a division of Dialysis Clinic, Inc. (DCI) that is responsible for leading healthcare innovation, including testing and implementing new ways to improve care for people with kidney disease. We are a passionate and hard-working team that seeks first and foremost to carry forward the mission of DCI: “The care of the patient is our reason for existence.”

The REACH Kidney Care Administrative Support is based at the corporate office in Nashville, TN. You will be working with staff in REACH Premier locations in the field, and will need to build a collaborative relationship with other members of the REACH team whom you will not get to interact with face-to-face on a daily basis. You will have the opportunity to support the continued growth of the program by ensuring that patients who would like to enroll in REACH services are able to do so with the REACH care team. You will interact with patients regularly over the phone and provide them with basic information about the program and help them understand that the REACH care team is here to promote their health while helping them navigate kidney disease. You will also support the team by gathering, recording, and sharing important patient information to enhance the quality of care we provide to patients.

Responsibilities

  • Process referrals and enter into the electronic medical record (EMR)

  • Contact patients for initial, follow up and missed appointments

  • Meet goals for appointments set per day

  • Document all attempts to contact patients

  • Call patients to remind of upcoming appointments

  • Draft and mail any needed written outreach for patients

  • Request medical records and confirm receipt of medical records ahead of patient appointments

  • Enter key information into the EMR such as patient demographics, problems, lab values, etc.

  • Manages telephone calls to the REACH Premier programs

  • Continually identify opportunities for improvement regarding a high quality and efficient workflow

  • Maintain organized records and workflow

  • Attend team meetings when appropriate

  • Work with members of the clinical and operations team to provide essential support

Qualifications

  • HS degree or equivalent

  • Strong working knowledge of Microsoft Office suite

  • Ability to adapt to new software

  • Excellent external-facing and internal communication (both verbal and written) skills

  • Attention to detail and commitment to excellent and reliable work
