Overview
This position will be remote, with apx. 20% travel.
REACH Kidney Care, a division of Dialysis Clinic, Inc. , is a kidney health management program designed to benefit patients along the continuum of kidney disease through health education, self-management, behavior change counseling, coordination of care with other providers, and patient navigation services.
The REACH Health Plan Partnership Manager will have a key role in building and maintaining long-term partnerships with health plans. REACH approaches transforming kidney care from a partnership perspective. Building long-term partnerships with health plans and payers to promote the health of their members with advanced kidney disease and demonstrate total cost of care savings through care management services is a foundational element for REACH.
The Health Plan Partnership Manager will work in partnership with the REACH business development team as they begin building a relationship with potential health plan partners. You will seek to learn the strategic direction of the health plan, understanding their business needs and how REACH can best meet their needs. As REACH implements services with a partner health plan, you will remain a constant in the relationship to provide continuity. After implementation, you will take the lead as the day-to-day point of contact with the health plan.
Responsibilities
Learn the needs and strategic focus of each health plan partner
Gain insight into how REACH can build synergy with health plans to meet their strategic needs and deliver value while improving care
Regularly solicit feedback from health plan partners
Manage communication day-to-day with health plan partners
Provide key reports and information to health plan partners
Take the lead on creating presentations to health plan partners
Facilitate meetings between REACH and health plan partners
Measure and regularly report on key performance outcomes related to health plan partnerships, identify opportunities for improvement, and collaborate on achieving improvements
Interface with operations and clinical team to share feedback and strategic insights toward continuous improvement and evolution of REACH
Other duties may be assigned as necessary
Qualifications
Bachelor’s degree, master’s preferred
3-5 years of experience working with health plans
