Dialysis Clinic, Inc. REACH Kidney Care Social Worker-MSW in Nashville, Tennessee


REACH Kidney Care is a wholly owned subsidiary of Dialysis Clinic, Inc. The REACH Social Worker will play an important role in improving care for people with advanced kidney disease both at a population level as well as at an individual level. To support the REACH care team, the REACH Social Worker will assess the psychosocial needs of advanced kidney disease patients and deliver targeted interventions for social, emotional and environmental issues that impact a patient’s ability to improve their health and benefit from provided care.

At the corporate level, you will work closely with the clinical and operations teams to address psychosocial needs for people with advanced kidney disease, including advanced CKD, kidney transplant, ESRD on dialysis and end-of-life. You will approach this work with a mindset of continuous improvement and will implement quality improvement initiatives as needed. You will design training curricula for nurse care managers and other members of the care team to build expertise in addressing social determinants of health and behavioral health needs. You will compile a library of tools and resources for the care team to utilize in working with patients individually and design educational materials for patients. In addition to addressing psychosocial needs at the population level, you will dedicate time to consulting with patients individually to provide your expertise in addressing their complex needs.


  • Work as a member of the corporate clinical team to provide central resources to the care team working with patients individually

  • Design training curricula for staff and educational materials for patients

  • Build and expand a library of psychosocial community resources for each local care team

  • Complete psychosocial assessments and assist with the development of the plan of care and progress toward goals.

  • Provide targeted interventions to meet identified patient needs.

  • Meet with individual patients in a telemedicine format (eg: Zoom) to address their psychosocial needs

  • Collaborate with other members of the care team to facilitate a comprehensive approach to care that aims to engage and empower patients and improve care.

  • Refer patients to community or other resources as needed.

  • Participate in organization-wide quality improvement initiatives, including the identification of opportunities to better meet the psychosocial needs of the population.

  • Act in compliance with REACH policies for clinical care and documentation.


  • Master’s Degrees from an accredited social work program.

  • Current state clinical license as a social worker. REACH will support you in obtaining licenses for the appropriate states where you will consult with patients individually.

  • Experience with chronic kidney disease or chronic condition population preferred.
